Overview

A series of trays, each one slightly different.

Your teeth are scanned, the movement is planned in software, and a laboratory prints a sequence of trays. Each one is very slightly different from the last, so wearing it applies gentle pressure that moves specific teeth a fraction of a millimetre. Small tooth-coloured attachments are bonded to some teeth to give the trays something to push against. You change trays roughly weekly and see the provider every six to ten weeks.

  • They work well for mild to moderate crowding, spacing, and relapse after previous braces.
  • They struggle with severe rotations, significant bite correction, and moving molars any real distance.

Two lines decide whether a quote is honest: whether refinements are included, and whether retainers are included. Those two together can add $1,500 to a headline fee that looked competitive. Get both in writing before you pay a deposit.

What insurance usually covers

  • Orthodontic cover is a lifetime maximum of $1,000 to $2,500 on plans that include it — not annual.
  • Many plans cover under-19s only. Adult aligner cover exists but is much less common.
  • Aligners and braces are usually treated identically by insurers, so choosing aligners rarely reduces your benefit.
  • FSA and HSA funds cover the whole thing and are frequently the better route for adults with no orthodontic benefit.

Mild to moderate crowding, spacing, and relapse after previous orthodontics are the sweet spot, and aligners handle those very well. Severe rotations, teeth that need moving vertically, significant bite correction and moving molars any real distance are where they struggle. A 3D scan will tell you within a week — and a provider willing to say "braces would be better for you" is worth far more than one who says yes to everything.

Twenty to twenty-two, which means everything except eating, drinking anything other than water, and cleaning your teeth. This is not padding: each tray applies a planned force for a planned duration, and wearing it for fourteen hours a day means the teeth do not reach the position the next tray assumes. That mismatch compounds and is by far the most common reason treatment runs long.

They measure how many cases a practice submits to the manufacturer each year — they are a sales volume metric, not a clinical quality rating. A high-tier general practice may have done a thousand simple cases; a lower-tier orthodontist may handle far more difficult ones. Look at case volume alongside whether an orthodontist is involved, which is the more informative pair.

They are considerably cheaper and they carry real risks. Most involve no in-person examination and no X-rays, which means undiagnosed gum disease or decay can be present while teeth are being moved, and root resorption can go unnoticed. They also cannot use attachments, which limits them to very mild front-tooth movement. For a genuinely tiny correction in a healthy mouth some people do fine. It is a gamble with your own teeth as the stake.

At conversational distance, rarely. Close up, the attachments — small tooth-coloured bumps bonded to some teeth — are visible if someone is looking. The trays themselves can pick up staining from coffee, tea and red wine, which is why you remove them for anything but water. Most people report a slight lisp for the first two or three days that disappears entirely.

A short additional series of trays at the end to fine-tune anything that did not land exactly as planned. Most cases need one round; some need two. It is normal rather than a sign anything went wrong. What matters is whether refinements are included in your fee — this is the single most common hidden cost in aligner treatment, so get it in writing.

Yes, indefinitely. Teeth drift throughout life and freshly moved teeth drift fastest in the first year. A thin wire bonded behind the front teeth plus a removable night retainer is the most reliable combination. Ask whether retainers are included in the treatment fee — like refinements, they are frequently excluded from the headline number.

At the end rather than during — and the final aligners can often be used as whitening trays, which is a genuine convenience. Whitening during active treatment is uneven because attachments cover parts of the tooth surface. Plan whitening for the last few weeks, and if you are also having veneers or crowns, whiten before those are made.
